Job Description: Reporting directly to the Manager, The Social Worker  BSW has responsibility for professional psychosocial assessment and counselling with clients and families who are managing social and emotional impacts associated with the complexities of various cardiac diagnoses, including, but not limited to, heart failure, coronary artery disease, atrial fibrillation. The Social Worker  BSW provides services as a member of a multidisciplinary team, working collaboratively with the care team, community groups, agencies and other professionals within the community.

- Bachelor of Social Work (BSW)
Knowledge and Abilities
- Ability to work cooperatively in an integrated interdisciplinary team
- Systems Theory/Family Systems Theory
- Knowledge of assessment, triage, treatment and case management.
- Providing services to patients/families using a variety of treatment modalities
- Extensive knowledge of Saskatchewan based services supporting cardiac patients
- Strong interpersonal skills
- Knowledge in principles of patient and family centered care
- Makes independent clinical judgment based on evidence based best practice
- Ability to work with a variety of individuals presenting with complex issues
- Demonstrated knowledge in the area of assessment and referral coordination
- Expert knowledge and advanced skill in a variety of treatment modalities
