Job Description: Reporting to the manager of Access Place, the RAAM clinical nurse who integrates experience and advanced level skills in the provision of direct patient care. This position assists the Addictions Medicine physicians and the infectious disease specialists in the provision of patient care as required and also assists the HIV and Hep C Nurse Clinicians in all aspects of clinical and outreach service and activities. The The RAAM clinic nurse position requires skill in collaborative multidisciplinary consultation particularly with the clinician, the case manager and the broader outreach team. The position will ensure seamless coordination of clinical planning, therapeutic activities, and client interventions. The main focus of this position is the implementation and delivery of care for individuals utilizing the Harm Reduction Programs including RAAM clinic services, Opioid Replacement Treatment, Hepatitis C care, Positive Care Program, and the Sexual Health Clinic.
