The CDS Intake Coordinator provides support to the Cognitive Disability Strategy (CDS) program by: creating meeting agendas, processing CDS intake applications, and entering data for statistical purposes. The CDS Intake Coordinator is responsible to answer stakeholder inquiries as needed.
Duties and Responsibilities
Services
* Collaborates with CognitiveDisabilityStrategy (CDS)applicants and their teamstofinalizeincoming applications to ensure all required information is attached.
* Manages and completes monthly CDS Intake Agendas and notifiestheCDS consultantsin a timely mannerwhenit iscomplete.
* Requests Daily Living Support Assessments (DLSAs) and emails toSaskAbilities.
* Manages the correspondence in relation to the CDS email inbox.
* Administration of all CDS documentation including incoming/outgoing letters, applications.
* Generates meeting minutes and sends CDS applications to CDS Provincial Office.
* Creates annual meeting schedules and makesappropriate bookings.
* Management of provider invoices.
* Attends CDS intake meetingsmonthly, as well as CDS consultant meetings as needed.
* Perform other duties as assigned.

Management of Resources
* Reconcile and audit account receivables for CDS program as required.

The incumbentisresponsible forcompleting all mandatory training requirements for the position as per agencypolicy andguidelines.
