Support Worker will provide structured support, individualized case management, along with community-based programming to foster skill building, recovery, and social connection.
Duties:
- Conduct a needs assessment to determine the level of programming such as daily life skills, community activities and transportation.
- Develop and implement case management plans for clients, including crisis intervention, individual and group counselling, and referral to other resources.
- Collaborate with other professionals to ensure clients receive comprehensive support.
- Keep accurate and up-to-date records of client contact, support and action plans, and progress using case management techniques.
Qualifications:
- Bachelors degree in Social Work or related field of study and/or a suitable combination of education and extensive lived experience.
- Ability to speak Metis or First Nations language is an asset.
- Knowledge and understanding of the history of Aboriginal peoples and their socio-economic backgrounds.
- Excellent communication skills, verbal and written to effectively communicate with clients.
