Stapleford Medical Clinic is seeking a skilled and compassionate Registered Nurse (37.5 hrs/week) to provide patient-centred diabetes care, education, and ongoing clinical management for individuals with prediabetes, Type 1, and Type 2 diabetes. This role focuses on empowering patients through evidence-based education, lifestyle counselling, medication support, and continuous follow-up to improve long-term health outcomes.
Working collaboratively within an interdisciplinary team, you will build strong patient relationships, support behaviour change, and deliver high-quality chronic disease management. This role is part of a Saskatchewan Innovation Fund initiative to enhance access to team-based primary care. Temporary until March 31, 2027, with possible extension.
Key Responsibilities:
Conduct comprehensive diabetes assessments and risk evaluations
Provide individualized education (glucose monitoring, insulin use, nutrition, activity)
Support medication initiation and adjustments per clinical protocols
Monitor chronic disease indicators (A1C, BP, lipids) and update care plans
Deliver follow-up care, coaching, and group education (as applicable)
Collaborate with physicians and allied health professionals
Maintain accurate EMR documentation and IPAC standards
Qualifications:
Current CRNS registration and BScN (or equivalent)
BLS certification (ACLS an asset)
23 years experience in nursing; diabetes/chronic disease experience preferred
Certified Diabetes Educator (CDE) strongly preferred
Strong clinical knowledge, communication, and coaching skills
EMR proficiency and commitment to patient-centred care
