Job Description: The Transplant Nurse Coordinator is responsible for the clinical & delegated administrative functions of the Extra-Renal (Out of Province) operations of the Saskatchewan Transplant Program. Providing assessment of client needs & clinical coordination for transplant patients pre/post-operatively. Responsible for monitoring & tracking patients on the transplant waiting list to assess for changes in clinical condition & communicate changes with the transplanting program. Assesses patient needs/concerns through chart review, history taking, & lab data in consultation with physicians. Provides education to patients/families in preparation for & follow up of transplant. The incumbent will provide other educational needs for nursing/physicians/staff as required. Identifies orders & prioritizes diagnostic tests & procedures in consultation with the physician. Participates in developing/monitoring/evaluation of Standard Operating Procedures in collaboration with physicians & other staff.

Experience
- Requires five (5) years recognized clinical experience in an acute care facility
Education
- Bachelor of Science in Nursing (BScN)
Other Education and Training
- Nursing diploma is acceptable
- Basic Life Support Certificate (BLS)
