This position involves providing support to 52 residents with severe and persistent mental illness, and will have a caseload of 20-25 PALS residents. In this position, you will be working under the direction of the PALS team lead, and overall direction of the PALS and OAK Supervisor with the goal of providing case management and everyday support services. This can include but is not limited to things such as taking residents to appointments, shopping, meal preparation, social/recreational activities, facilitating educational groups, and assisting with budgeting, monthly reporting, conducting assessments and compiling annual Wellness Plans.
Additionally, they are responsible for providing one-to-one counselling, in the areas of mental health cognitive strategies, personal safety strategies, goal setting, problem solving, problematic substance use, relapse prevention, etc. Shift work required. Must have a valid drivers license.
This position is a full-time/permanent position that includes day shifts from 8AM-5:00PM.
Post-secondary education required in a related field (psychology, sociology, human justice, Social Work).
