Description

We are seeking a highly motivated and compassionate individual to join our team as an ICC. As an ICC, you will be responsible for providing comprehensive case management services to individuals from vulnerable populations. This is an opportunity to make a difference in the lives of individuals in need while working with a dedicated team of professionals. Reporting directly to the Housing Programs Manager, the ICC is responsible for the provisions of housing stabilization and intensive case management services for male clients that are housed in a 24-hour 7 day a week supported housing environment. This is for a M-F 4pm-midnight FTE position.
Job Duties:
Provide in home life skills mentoring (including but not limited to budget planning, meal planning).  
Work alongside other NYFC program staff to case plan and case manage clients to ensure they have access to a wide array of supports promoting a holistic approach to over-all wellbeing.
Work in a team environment to promote functions and cleanliness of the home.
Assist other program support staff as needed to ensure all programs are operating efficiently.  
Maintain and report on program statistics and outcomes.
Adhere to organizational policies and procedures.
Helping clients achieve wellness and autonomy to live independently in the community.
Conduct comprehensive assessments of clients' needs and develop individualized plans of care.
Collaborate with other service providers to ensure coordination and delivery of necessary services.
Provide crisis intervention and support to clients during difficult situations.
Monitor clients' progress and make appropriate referrals to additional resources as needed.
Advocate for clients' rights and work towards their empowerment.
Maintain accurate and up-to-date documentation of client interactions and progress.
Participate in team meetings and collaborate with colleagues to develop effective strategies for clients.
