Description

The Family Support Case Worker provides trauma-informed, relationship-based support to mothers and children at Hopeful Hearts Home, focusing on strengthening the motherchild relationship and addressing the complex emotional, behavioral, and developmental needs of children impacted by interpersonal violence. This role includes assessment, advocacy, case management, and individualized service planning, along with facilitating therapeutic, play-based, and parenting-focused programming that supports healing, stability, and resilience.

MAJOR RESPONSIBILITIES:

1. Program Coordination and Support
Support mothers and children using trauma-informed, attachment-focused, and play-based approaches
Provide parenting support through coaching, observation, and problem-solving
Facilitate therapeutic, skill-building, and family-centered programming

2. Case Management
Assess needs and implement individualized service plans
Provide referrals, advocacy, and coordination with community partners
Maintain accurate documentation

3. Team Collaboration
Collaborate with team members to ensure consistent service delivery
Support overall program operations

QUALIFICATIONS:
Bachelor of Social Work or equivalent combination of education and experience
Minimum two (2) years experience supporting children and families impacted by trauma/violence
Experience with case management, assessments, and program facilitation
Strong understanding of trauma-informed care and child development
Valid drivers license and reliable vehicle
Criminal record check with vulnerable sector
