Description

MGBHLM is seeking a full-time high school home liaison. The home liaison supports students and families by strengthening the connection between home, school, and community. This role focuses on improving attendance, engagement, credit attainment, and successful transitions for high school students both on- and off-reserve. The Liaison provides direct, culturally responsive support and works collaboratively across programs to remove barriers and improve student outcomes from Grade 8 through post-secondary pathways.

Responsibilities:
Student and Family Support
* Build and maintain strong, respectful relationships with students and families
*Conduct home visits and regular check-ins to support student success
* Monitor attendance, engagement, and academic progress
* Advocate for students within school systems on- and off-reserve
* Maintain accurate documentation, case notes, and reporting
Grade 89 Transition Supports
*Coordinate transition planning for Grade 8 students entering high school
* Organize school visits, orientations, and student shadowing opportunities
* Support course selection and high school registration
* Identify at-risk students and develop individualized transition plans
* Provide consistent follow-up during the first year of high school
* Work with families to address barriers such as transportation, supplies, and attendance expectations
High School to Post-Secondary Transition Supports
* Support career exploration and post-secondary planning
* Assist with applications to post-secondary institutions such as First Nations University of Canada, Saskatchewan Indian Institute of Technologies, and North West College
* Provide guidance on funding applications including PSSSP, band funding, scholarships, and bursaries
* Assist with required documentation such as transcripts, identification, and acceptance letters
