Experience
- 18 months of recognized (postgraduate) clinical experience preferred
Education
- Recognized degree in nursing , or a social work degree.
Licenses
- Valid Class 5 driver's license

- College of Registered Nurses of Saskatchewan (CRNS)
- Saskatchewan Association of Social Workers (SASW)

- Knowledge specific to populations being served, example: Gerontology & Aging
- Knowledge of community resources
- Demonstrated ability to practice core functions of case management
- Openness to teaching and providing educational opportunities.
- Ability to facilitate case/family conferences.
- Effective interpersonal relationship skills
- Ability to communicate effectively both orally and in writing
