Position Summary:
Reporting to the Director of Client Services, this individual will provide direct client care and medication counselling, chronic disease management, pharmacy support to the acute care unit, and act as a resource to the primary health care providers and community health programs. The role of the Pharmacist will include, but is not limited to:
- Comprehensive medication review in relation to clients at risk and/or living with Chronic Kidney Disease
- In collaboration with RQHR rural pharmacy services, provide medication management for the acute care unit.
- Identify and resolve actual/potential drug related reactions to prioritize medication safety and efficacy
- Liaise with patients, family members, cultural staff and other healthcare professionals in the provision of medication and traditional medicine education, support and counselling
- Facilitate prescription management, compliance aid, drug coverage and other issues as required
- Participate within the interdisciplinary team in expanded project development, community initiatives and research
Qualifications:
- Bachelor of Science in Pharmacy Degree from an accredited university
- Current Registration with the Saskatchewan College of Pharmacy Professionals
- Certificate of completion of an Accredited Hospital Pharmacy Residency or suitable experience
Experience:
- Minimum 3 years' experience
- Experience working in an acute care setting an asset
- Previous experience working within a community setting with Indigenous clients
Hours of work: 8:30am to 4:30pm (weekdays, with some flexible hours required)
Rate of Pay: Salary to commensurate with qualifications and experience
