Description

The Wrap Around Service Facilitator will provide person-centered case management to tenants identified as needing intensive support in maintaining their housing. The facilitator will be responsible for identifying, coordinating, and facilitating a support team to assist tenants in the stabilization and maintenance of housing, as well as the development of independence, self-sufficiency, and self-esteem.

Responsibilities include:
-Respond to questions by phone or in person regarding the Wrap Around Service Program
-Attend home visits with tenants
-Assess for eligibility
-Perform intake assessment
-Include community based resources in case planning
-Assist tenants with completing documentation
-Document, monitor and assess progress of the tenant regularly
-Develop a transition plan
-Report and document outcomes
-Other duties/responsibilities as mutually agreed
Required Knowledge and Skills:
-Understanding of holistic and strength based practices
-Knowledge of municipal, provincial, and federal programs/services and the eligibility requirements for each program/service
-Knowledge of intake procedures/interviewing techniques
-Skill in interacting with people of different social, economic, and ethnic backgrounds
-Skill in establishing and maintaining effective working relationships
-Ensures a consistently high level of organization
-Ensures privacy and confidentiality at all times
Additional Requirements:
-Must be comfortable attending home visits alone to meet with tenants to address immediate and ongoing concerns/needs
-Must transport tenants in personal vehicle if necessary for case plan or safety reasons
-Must obtain the required insurance for transporting tenants
Qualifications:
-University Bachelor's Degree in Social Work or a closely related field
-1-2 years case management experience
-Possession of a valid driver's license and a private vehicle
-Satisfactory Criminal Record Check
