Job Description: As a member of the interdisciplinary team and under the general direction of the Out-of-Scope Manager and assigned Clinical Supervisor, this co trained individual is responsible for the day to day client service delivery (development, implementation, delivery and assists with evaluation) of Addiction Services.  Performs clinical case management including work with co-occurring mental health disorders, acting as a triage unit to refer to the appropriate agencies; community education and community development.  This position will be responsible for providing services to assigned PAPHR Health Care Facilities and schools.  This positions is located in Prince Albert, since some travel in rural areas may be reqiured a valid drivers licence and access to a vehicle will be required.

Required Qualifications

Valid Class 5 driver's license
Bachelor Degree in Chemical Dependency studies or another Bachelor Degree acceptable to the employer
Knowledge, Skills and Abilities

Interpersonal skills
Counseling skills
Organizational skills
Knowledge of the development model of recovery (SMRS)
Effective oral and written communication skills
Knowledge of addictions and their effects
Ability to accurately assess addictions and make appropriate recovery plans
Demonstrated knowledge of a holistic addiction treatment model and the developmental model of recovery (DMR)
Demonstrates the ability to prioritize, interpret and respond appropriately to issues.

Delivery of approximately 16-20 hours per month of educational and clinical tobacco cessation services to PAPHR programs and staff as required. 56 hours of work in a 3 week rotation. End date subject to
