Job Description: As a member of the interdisciplinary treatment team, the Behavior Support Coordinator is accountable for the provision of timely consultative services, including assessment and case management for "difficult to serve"  clients residing in the Long Term Care facilities, and receiving services from the Community Based Home Care Program, in Heartland Health Region.  The incumbent collaborates with regional LTC and home care staff.  The incumbent will coordinate all information related to behavior management issues.  He/she will assist HRHA LTC and home care staff to utilize quality indicators to improve the quality of care and quality of life for clients with behavioral problems.
Required Qualifications

Valid Class 5 driver's license
Registered or eligible for registration with the Registered Psychiatric Nurses Association of Saskatchewan (RPNAS)
Degree or Diploma from a recognized Psychiatric Nursing Program
Experience working with geriatric clients would be considered an asset
Experience
Demonstrated experience in working with clients with psychiatric diagnosis.  Related post-secondary education is an asset.  Experience working with individuals with challenging behavioral issues is an asset.
