Job Description: Primary Health Care is a delivery model that utilizes integrated, interdisciplinary teams of health professionals and community members to provide high quality service as close to home as possible, at the most appropriate time, by the most appropriate person.  The Primary Health Care Team Manager - Meadow Lake reports to the Director of Primary Health Care and is accountable for the PHC team members in the Meadow Lake, Goodsoil and Loon Lake areas. This position provides leadership in team building, clinical excellence and interdisciplinary team practice.

Required Qualifications:
Degree in Health Sciences
Proficient with computer software (MS Word, PowerPoint, Excel, Outlook).
Excellent written and verbal communication skills for individual, small group, and large group interaction both within the region and the community.
A valid driver's license required.

Experience:
Leadership and/or supervisory experience is required.
Experience working with a PHC team is an asset.

Geographic Location: Meadow Lake
Expected Start Date: February 5, 2018
FTE: 1.00

Hours of Work: In a 1 week rotation: 5 shifts of 7.50 hours
