Job Description: The Home Care Manager, under the general direction of the Director of Home Care is responsible for the overall management of the home care program within the geographic assessment area assigned. As a team leader the Home Care Manager will direct the Home Care staff in the most cost-effective manner to deliver the following programs: Assessment, Care Coordination, Scheduling, Procure, MDS-HC and Home Care Accounting. The Home Care Manager is also accountable for all in office staff as well as supervising, evaluating and directing field staff in clients' homes. Other managerial functions will include but may not be limited to, case management, assessing client needs, hospital discharge planning, developing and coordinating care plans as well as being the clients advocate, overseeing scheduling, billing, payroll functions.
Experience
Experience in Home Care.
Experience in administrative or supervisory capacity an asset.
Experience in Palliative Care an asset.
Experience with computer programming an asset.
A suitable combination of training and experience may be considered.
