Description

The Case Manager will work to support individuals who have been involved in a high risk lifestyle, with the philosophy of building on strengths and supporting challenges. This will require providing one on one support as well as working with various professionals and support agencies to coordinate meeting the daily living, health, financial and social needs of individuals and/or families who have been involved in high risk lifestyle.
