Facility: Saskatoon City Hospital
Job Description: The Clinical Coordinator, Rehabilitation Day Services is an integral member of the Rehabilitation team. Under the direction of the Manager of Interprofessional Practice the Clinical Coordinator coordinates the services of the Rehabilitation Day Services multidisciplinary outpatient program.
Required Qualifications

Bachelor Degree/Diploma in Nursing, or Bachelor Degree in Occupational Therapy, Physical Therapy, Speech Language Pathology, Social Work, or other health related field
Knowledge, Skills and Abilities

Basic computer skills
Ability to adapt to changing conditions
Ability to work independently and recognize the need for consultation
Requires concentration and attention for resident/patient assessment, treatment, teaching and report writing
Requires coping skills to work with residents/patients dealing with stress, role loss and other psychosocial issues
Excellent interpersonal, verbal and written communication skills
Ability to function independently and exercise good judgement in decision making
Must be able to plan, organize and coordinate efforts with allied health members
Ability to perceive the broad scope of the clients involvement in and return to life roles
Responsible for accurate documentation of client related documentation
Knowledge of client case management and process facilitation
Ability to establish and maintain effective working relationships with a wide variety of professionals, public agencies and families
Other Information

Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement.  HSAS senior
