Job Description: Reporting to the Manager of Chronic Kidney Disease, the Home Hemodialysis/Peritoneal Dialysis Patient Educator responsibilities will include: Coordinate and participate in patient education for chronic kidney disease patients requiring peritoneal dialysis and home hemodialysis. This will include liaison with families, physicians, dietitians, staff nurses, and community agencies. Provide individual outpatient education for patients and families in a clinic setting. Promote continuing staff nurse involvement in patient education through effective communication, development of resource materials and provision of in-service education. Coordinate and participate in education programs for designated nursing staff that will be providing peritoneal dialysis for patients at SPH and other agencies. Serve as a primary clinical resource for nursing, medical and other professional/support staff. Participate in departmental activities (program development, planning, quality improvement, and evaluation). Participate in research and evaluation projects.
Required Qualifications
Bachelor Degree in Nursing
Basic Life Support (BLS)
Registered or eligible for registration with the Saskatchewan Registered Nurses Association (SRNA)
Experience
3 years related clinical nursing experience with patients and families with chronic disease, in particular chronic kidney disease required.
CNA Certification in clinical practice area is preferred.
Experience with Peritoneal Dialysis and Hemodialysis Preferred.
Masters Degree in Nursing, Adult or Continuing Education is an asset.
