Job Description: Reporting to the Manager, Chronic Disease Management, this positions works as part of a multi-disciplinary team within the heart function clinic in Chronic Disease Management. The nurse will provide a variety of services required by inpatients, outpatients and satellite departments.
Required Qualifications

Bachelor Degree in Nursing
Basic Life Support (BLS)
Registered or eligible for registration with the Saskatchewan Registered Nurses Association (SRNA)
Experience
1 YEAR EXPERIENCE IN ADULT EDUCATION AND GROUP FACILITATION RELATED TO THE AREA OF CARDIOVASCULAR AND DIABETES DISEASE MANAGEMENT REQUIRED.  1 YEAR EXPERIENCE IN PROGRAM/RESOURCE DEVELOPMENT RELATED TO NEEDS ASSESSMENT, PLANNING, PROGRAM DESIGN, IMPLEMENTATION AND EVALUATION, TEACHING/COUNSELLING, FACILITATION SKILLS AND LEADING GROUP ACTIVITIES REQUIRED.  3 YEARS RELATED CLINICAL NURSING EXPERIENCE WITH PATIENTS AND FAMILIES WITH CARDIOVASCULAR AND DIABETES DISEASE MANAGEMENT REQUIRED.
Knowledge, Skills and Abilities

Ability to competently use computers and software applications as required by the position
Advanced assessment skills
Communication skills to transfer knowledge/skills

NOTE: This is a Provincial Posting and is open to all SUN members within the bargaining unit as per Articles 27.02 and 27.05 of the SUN Collective Bargaining Agreement. First preference will be given to employees of the SKHA (Saskatoon) and its affiliated agencies.

Hours of Work: Shift Information: Days
In a 3 week rotation: 14 shifts of 8.00 hours
