Job Description: Community Care is a delivery model that utilizes integrated, interdisciplinary teams of health professionals and community engagement/partnerships to provide high quality service as close to home as possible, at the most appropriate time, by the most appropriate person. The Health Educator Senior reports to the Manager,Community Care and is accountable for the development, implementation, facilitation, evaluation, and research of local community care initiatives.
Required Qualifications

Relevant education/experience related to team facilitation, community development, health systems, policy development, and evaluation and research
Degree in health or social related field
Experience
Minimum of two years experience in a Community Care setting preferred e.g. participation on a Community Care team; experience with collaborative team functioning, experience with community participatory activities.
Knowledge, Skills and Abilities

Ability to work independently and as a member of a multi-disciplinary team
Communication skills
Ability to competently use computers and software applications as required by the position
Ability to travel both within the region and provincially
Facilitation skills
Mediation skills
Presentation skills
Proficient in computer software (Word, Power Point, Excel, Access, Outlook)
Excellent written and verbal communication skills for individual, small group, and large group interaction both within the Region and the Community
Basic training in conflict resolution, mediation, coaching and Lean would be an asset
Ability to integrate computer knowledge with the need for team communication, enhanced community partic
