Your role of Support Services Coordinator will be to improve access to information, intake, and service delivery so that people are able to easily navigate and experience a seamless and co-ordinated system of disability programs and services.
This includes providing individuals and families with case management and support services to meet their needs including:
Facilitating funding applications and/or acting as the case lead to coordinate meetings and provide written reports to team members.
Supporting individualized school-based teams or community based programs.
Providing a central location of information regarding existing services and supports and maintain ongoing communication with these services
Creating and arranging inclusive social and recreational opportunity programs for children, youth, and adults with special needs and the supervision of HDCS's annual Summer Program.
Assist families in accessing caregivers appropriate to their respite and mentorship needs.
A Bachelor Degree in Social Work is preferred.
