Under the supervision of the Director of Judicial Programs, or her designate, Reintegration Caseworker will provide direct services for hard to serve adults who are incarcerated and/or on probation orders within the community.
Duties
Assist in providing life skills development, including:
Promote positive independent problem solving & decision making skills
Provide info & life skills support to clients
Teach self-advocacy
Provide crisis intervention & behavioral support, including:
Provide stabilization crisis support
Provide incidental problem solving & emotional support to clients
Provide emergency behavior management
Provide non-emergent behavioral management to clients
Assist in case management & support for clients, including:
Implements risk reductive case plans
Assist clients in accessing services with other agencies
Make referrals to other professionals as necessary
Provide institutional support, including:
Working in partnership with Correctional Staff within Correctional Facilities
Answering the support phone line
Providing support for incarcerated adults
Seeks out new resources within community for clients
Liaison with Federal and Provincial correctional facilities
Provide risk relapse prevention programming, including:
Provide info & risk relapse prevention programming to clients to assist in decreasing risk and increase self-awareness & manage triggers related to clients criminal cycle
In addition:
Role model positive behavior/encourage and assist clients to practice self-help & non-violent problem solving
Ability to provide effective & accurate written documentation
Ensure the services being provided are risk reductive, individual, integrated, & client orientated
Assist in developing & maintaining risk reductive and emergency response plans and personal safety plans
Report all incident of suspected abuse to appropriate authorities
Perform other related duties as assigned
