As part of a collaborative multidisciplinary team and utilizing an outreach approach the Case Manager provides addiction and mental health counseling and treatment support to those who are accessing services from the OARS Clinic including psychosocial assessments on intake, addiction assessments, client goal setting and skill building, and coordination and support of health and social services. In addition, provide short term, solution focused counselling to clients at the Battlefords Family Health Centre as referred by the mediacl team practitioners.
The Case Manager will have sound knowledge of HIV, Hep C and addiction recovery with Methadone as part of the recovery process, knowledge in concurrent disorders, clinical practices in addictions, case management, referrals, counselling and assessment. Requires ability to interpret interpersonal and intercultural dynamics and implement therapeutic strategies to ensure appropriate treatment planning. Skills in program planning, collaboration, communication and leadership are required.
Qualification and Experience:
* A minimum of a Bachelors Degree in Social Work or related discipline with related experience
* A minimum of one (1) year experience working with marginalized populations affected by addictions in a health-related field
* Knowledge of the Chronic Disease Management and behaviour change models, addictions, harm reduction theory, teaching/learning principles and motivational interviewing
* Demonstrated knowledge of community resources and programming supportive of this client population
* Demonstrated skill in counselling modalities such as Cognitive Behavioral Therapy, Motivational Interviewing or Brief Therapy would be beneficial
* Demonstrates knowledge and understanding of traditional aboriginal values, beliefs and healing approaches
* Incumbent is self-directed and requires minimal supervision
* Successful criminal record with vulnerable sectors check
