Job Description: Reporting to the manager of Chronic Disease Management, in partnership with the Department of Pediatrics the responsibilities include; Coordinate education for children and their families living with diabetes; provide best practice in diabetes management as part of a multidisciplinary team working closely with regional diabetes programs in the province; provide individual inpatient and outpatient education for children and families; facilitate group classes for outpatients; promote team involvement in self-management education; participate in departmental activities such as program development, planning, quality improvement and research.
Required Qualifications
Bachelor Degree in Nursing
Basic Life Support (BLS)
Membership in the Diabetes Educator Sector of the Canadian Diabetes Association
Registered or eligible for registration with the Saskatchewan Registered Nurses Association (SRNA)
Experience
Experience working with children/families with diabetes.
Experience working with children/families to enhance self-management skills.
Experience in pediatrics and as a diabetes nurse educator is preferred.
Minimum of three(3) years related clinical nursing experience with children and families with diabetes.
Minimum of one (1) year experience in adult/pediatric education and group facilitation related to the area of diabetes.
Minimum of one (1) year experience in program/resource development related to needs assessment, planning,program design, implementation, evaluation, teaching/counseling, facilitation skills and leading group activities.
