Description

Under the supervision of the Director of Social Programs, the APC will provide daily support for household operations and case management of JHSs Residential Programs, working within the Youth Centered Model Continuum of Care.

Duties
Daily oversight of the home including:
Meal planning, grocery shopping, & budgeting
Identifying maintenance & upkeep issues
Ensuring home/yard cleanliness

Lead in case planning & case management, including:
Various documentation
Assist in developing, carrying out, & following up the crisis plan
Implements case plans/monitors success
Makes referrals as necessary
Partner with local agencies to develop responsive support for residents
Operate as the main contact with the youths Ministry workers, school, counselors, and external agencies
Coordinate youth appointments with residential staff & external agencies

Complete administrative tasks, including:
Filling shifts when staff call in sick as needed
Preparing various Ministerial reports
Reconciliation of household spending twice a month
Provide after hours/on-call support as required

In addition:
Provide support & guidance to residential caseworkers
Adhere to policies and procedures laid out in the Residential Services Manual
Report accidents, injuries, & illness and complete necessary reports
Report incidents of suspected child abuse to appropriate authorities & follow organization protocols
Other duties as assigned

Qualifications
2+ years work experience in a residential setting, with experience in case management
First Aid/CPR
Suicide Intervention, Food Safety, Non-Violent Crisis Intervention training
Valid drivers license
Clean Criminal Record check

Knowledge/Skills
Working knowledge and experience working in a residential facility for at risk youth
Excellent communication, organizational, interpersonal, and time management skills
Exceptional writing and case management skills
Demonstrated skills in providing supervision to youth
