Job Description: The CDM Nurse Facilitator reports to the Primary Health Care Team Manager and works with an interdisciplinary team with a chronic disease prevention and management focus. This position has two main areas of responsibility.  First, the CDM Nurse Facilitator collaborates with the multidisciplinary team members, community volunteers and community members to facilitate and coordinate self-management support programs for persons living with chronic conditions. Secondly, the CDM Nurse Facilitator is responsible for assessing the learning needs, developing and providing clinical education and consultation for PHC team members.  In addition this position is responsible for tracking mandatory and professional education for the department.  

Required Qualifications

Bachelor Degree in Nursing
Basic Life Support (BLS)
Basic Life Support (BLS) Instructor Certificate
Registered or eligible for registration with the Saskatchewan Registered Nurses Association (SRNA)
Knowledge, Skills and Abilities

Advanced communication skills
Ability to adapt to changing conditions
Ability to competently use computers and software applications as required by the position
Ability to develop and deliver formal education programs
Ability to develop, evaluate and modify programming provided to clients
Assessment skills
Communication skills to transfer knowledge/skills
Facilitation skills




Must have obtained one or more of the following Chronic Disease Management certifications prior to employment:

1) CNA Certification in Cardiovascular Nursing or equivalent national certification in Cardiac Rehabilitation;

2) Pulmonary Educator with national cert
