The MCH/FOF position works collaboratively within the One Arrow First Nation Health Clinic and works in partnership with other clinic staff and clinic programs and other community partners to improve the health and social outcomes for; pregnant women, families with infants and/or preschool aged children. The position supports programs and service delivery with; pregnancy, baby, immunizations, breastfeeding, the FOF and MCH programs recognizes an integrated and holistic approach to wellness during and after pregnancy.
Perform minimum, weekly home visits, establish and maintain, a nonjudgmental, professional working relationship with, clients and their families and provide information on; programs, identified issues. Assist clients with; goal planning, meetings, transportation, and activities as needs identify. Facilitate workshops and programming. Act as liaison as needed. Assist Health Nurse with pre and post natal visits and care, infant and child immunizations.
Completing and maintaining; client files, worklogs, summaries, evaluations, workplans, yearly and quarterly reports and other documents as requested for compliance with Accreditation Canada and Policy and Procedure of One Arrow First Nation. Liaise with; internal and external agencies, develop and maintain working relationships with off reserve agencies/organizations. Attend; interagency or case management meetings as required. Establish and maintain list of available resource agencies and referral contacts.
Establish and maintain interagency relationships with community and urban program providers. Excellent communication and interpersonal skills (written and verbal). Self starter with initiative and thoroughness. The ideal candidate will be approachable, act as an advocate for, and have a strong commitment to; parent, child and family development.
