Job Description: The Interstitial Lung Disease Chronic Disease Management (CDM) Nurse Educator integrates in-depth clinical expertise and experience, knowledge of nursing theory and the principles of teaching and learning, in the provision of chronic disease education, and quality care for clients, families, health professionals and students. The ILD CDM Nurse Educator is responsible for assessing learning needs, developing, coordinating, implementing and evaluating self-management educational/counseling activities for clients, families, health professionals and students with Interstitial Lung Disease/general respiratory (cystic fibrosis, obstructive sleep apnea, asthma) diseases. The ILD CDM Nurse Educator collaborates with multidisciplinary team members, inpatient/outpatient/hospital departments/community agencies to establish partnerships, initiating referrals to community services as required. The ILD CDM Nurse Educator uses a consultative approach, implementing accepted standards and guidelines and current research findings to advocate for continuity and coordination of care. Teaching can occur in inpatient, outpatient, specialty clinics, physician clinics or community settings, with individuals or groups. The ILD CDM Nurse Educator participates in research, program quality monitoring and improvement related to Interstitial Lung Disease using National and International clinical standards of practice in collaboration with physicians and multidisciplinary team members.
Required Qualifications
Bachelor Degree/Diploma in Nursing
Basic Life Support (BLS)
Registered or eligible for registration with the Saskatchewan Registered Nurses Association (SRNA)
Experience
- Requires a minimum of 3 years recent full-time clinical experien
