Description

The Community Health Team Leader is a Registered Nurse and is an integral part of the Health Care Team.  Reporting directly to the Health Director the Community Health Team Leader will work in a collaborative role to provide leadership, management and support to the Community Health programs.  They will assume responsibility for the day-to-day management of the Community Health programs and staff as well as the coordination and integration of these programs to meet community needs.
General Responsibilities
1.Provide leadership and direction to the Community Health Programs.
a)Provide overall direction, orientation, training, evaluation and supervision to the nursing and support staff working in the Community Health programs
b)Work with the Community Health program staff  to develop and enhance the Community Health programs and ensure the integration of the programs
c)Assist with providing and/or organizing presentations and education offered through the Community Health and Communicable Disease programs
d)Will be responsible for implementing and delivering at least one of the community health program.
2.Provide Administrative Support
a)Participate in developing and monitoring program budgets, work assignments and schedules
b)Collect and submit the required month-end
Knowledge of:  Community Health Programs, nursing scopes of practice, community development & cultural competence.
Skills: Facilitation and presentation skills, ability to work well with others, decision making skills and ability to prioritize
Must have proficient computer skills and familiarity with Panorama
Must have a valid Class 5 drivers license;
Must have a positive attitude and strong interpersonal skills;
Self-motivated and able to work independently;
Pass a Criminal Record and Vulnerable Persons Clearance;
Ability to speak Cree is an asset;
