Facility: STURDY STONE CENTRE & RUH EMERGENCY
Job Description: The Transition Team Social Worker assists individuals who primarily present to the RUH Emergency Department experiencing suicidal ideation or who are post suicide attempt. Working collaboratively within a multi-disciplinary team including the RUH Emergency Department Physicians and/or Dube Physicians and the patient and/or support network, the Transition Team Social Worker's primary responsibility is to assist the patient in transitioning their care plan from inpatient treatment to the community; by developing a community stabilization strategy which may include referrals to community based resources. The incumbent may be requested to attend at the RUH Emergency Department and assist the RUH Emergency Department Psychiatry, psychiatric liaison nurses and emergency staff to increase patient flow. When not attending the RUH Emergency Department, the incumbent will work out of an office located in the Sturdy Stone Building.
Required Qualifications

Bachelor Degree in Social Work
Registered or eligible for registration with the Saskatchewan Association of Social Workers (SASW)
Valid driver's licence and access to a personal motor vehicle for transportation related to Region business
Experience
-  Minimum of 1 year full-time experience working with individuals suffering from mental health and addiction related issues.
-  Experience in working with suicide intervention strategies required
