Facility: MENTAL HEALTH OUTPATIENT
Job Description: The PACT (Police and Crisis Team) is a collaborative partnership between Mental Health and Addictions Services and Moose Jaw City Police. The goals of the project are to enhance public safety and care for individuals, families, and households with mental health issues through the most effective assessment, triage, and intervention beginning at the point of contact in the community. Under the direction of a Program Manager and as part of an interdisciplinary team, the Assessor Coordinator, SR.-PACT Team works as part of a Police and Crisis Team. The Assessor Coordinator provides a complex range of screening, triage, assessment, treatment, and case coordination services to an adult community mental health, addictions and/or forensic population. The incumbent provides program consultation and coordination, evaluation of clinical interventions, and provides professional and public education regarding mental health and addictions issues and resources. As a senior practitioner, acts as resource for other professionals for mental health and addictions in the community
Experience
Recognized degree in Nursing, Social Work, or another Health or Human Service field.
Registration with professional regulatory body.
Three years related experience ( post graduate) in health and social sciences with two years recognized experience working in a mental health and addictions service.


Knowledge Skills and Abilities
-Knowledge of a variety of Mental Health and Addictions concerns and interventions required to enable clients to sustain stability
-Knowledge of community resources is essential
-Considerable work experience with mental health and addictions community outreach and competence in delivering crisis interventions
-Demonstrated above average clinical assessment sk
