SUMMARY:
With direction from Willow Cree Health Services (WCHS), the incumbent will support and adhere to the Mission/Vision of the organization to provide services to the community members of Beardys & Okemasis Cree Nation (BOCN).
The Home & Community Care Registered Nurse (HCCRN) is responsible for providing, participating and directing staff in the delivery of a high standard of community health in accordance with Saskatchewan Registered Nurses Association (SRNA) scope of practice.
The HCCRN provides assessments for community band members needing assistance at home. The services provided are client and family focused including initial assessment, one-on-one teaching, treatment, palliative care, and rehabilitation to prevent deterioration and promote independence.
The HCCRN is part of the Health Care Team and is responsible for liaising between patients, families, the community and health care providers to ensure patients and families understand their conditions and treatment and to ensure the band members are receiving appropriate care. The goal is to minimize the effect of the disability and/or illness, support independence and wellbeing of individuals and provide their informal support and caregivers. The scope of the responsibilities of this position is to be a holistic approach to health and health care is delivered on an individual basis or as part of a multi-disciplinary team.
The HCCRN is a managerial role in which he/she will complete work plans, budgets, and reports in alignment with Health Canada funding. The HCCRN will provide supervision and administration duties to the HCC team and reports to the Director of Health.
