Job Description: As a member of both the Geriatric Evaluation and Management (GEM) team, and the Connected Community Care team, the Degree Recreation Therapist is an integral member of the interdisciplinary team providing support to community members with complex health needs. Services will be a mix of planned appointments, community outreach, and inpatient support for individuals on the Convalescent Unit (CU) at Saskatoon City Hospital. Services will be provided in a variety of settings that may include a community health centre, personal dwellings, seniors' high rises, personal care homes, enriched housing, hospital, etc. Accountable to the Manager of GEM, and working closely with the Connected Community Care Manager/Physician Dyad, the Recreation Therapist utilizes a strengths approach to practice with a focus on enhancing life-long leisure skills and attitudes, eliminating barriers to leisure and optimizing leisure participation for clients with chronic illness or disabling conditions. The clinical role includes assessment, intervention planning, utilization of evidence informed interventions, evaluation and documentation of Recreation Therapy services to those patients currently on CU, or eligible for services from the Connected Community Care team. The GEM and Connected Community Care Recreation Therapist is responsible for assessing and treating patients in assigned service areas, maintaining accurate patient documentation and statistical data, and participating in program planning required. In-service presentations to other staff and external agencies as required.
Required Qualifications

CPR certification
Must possess a valid driver's license and vehicle
