Job Description: -Under the general direction of the Manager or designate, provides an accessible, client-centered  and integrated approach to the processing of requests for Home Care services; performs initial consultation, preliminary assessment and identification of needs for potential clients; takes and screens referrals from various sources; determines eligibility of clients, initial interventions and the response time needed.
-This position is a new position as such requires the successful candidate to identify improvements needed in existing process; and to use creativity and resourcefulness to help improve response time, and ensure appropriate and effective client care in the community. Integrating new and improved intake procedures into the present environment will be a key component of the position.
-Utilizes case management strategies to develop client care plan.
-Working knowledge of the Procura Program and RAI HC assessment tool.
Required Qualifications

Bachelor Degree in Nursing
Basic Life Support (BLS)
Registered or eligible for registration with the Saskatchewan Registered Nurses Association (SRNA)
Valid Class 5 driver's license
