Job Description: Under the direction of the program area manager, the Palliative Care Coordinator designs, implements and evaluates palliative care services provided to clients in the Saskatchewan Health Authority.
Required Qualifications

Three years related experience in Palliative Care (combination of Acute and Community preferable)
LEAP certification (Learning Essential Approaches to Palliative and End of Life)
Current registration in good standing with Saskatchewan Registered Nursing Association (SRNA)
Must possess a valid Saskatchewan drivers license and have access to a reliable vehicle
Canadian Hospice Palliative Care Nurse Certification (CHPCN) recommended or willing to pursue when eligible
Baccalaureate Degree in Nursing
Knowledge, Skills and Abilities

Demonstrated commitment to continuing professional development
Ability to provide clinical recommendations to health care providers, clients and their caregiver
Proven ability to display a high degree of emotional maturity to interact with numerous patients and families in varying degrees of denial or acceptance of the medical outcome for themselves or their loved one
Proven excellent communication skills, both verbal and written
Ability to establish and maintain positive relationships with all stakeholders who are supporting palliative clients and families
Demonstrated ability to resolve problems and effectively manage conflict situations
Ability to demonstrate strong clinical assessment, decision making, and critical thinking
Proven ability to work independently, set priorities and show initiative`
Demonstrated ability to plan, organize and prioritize work within a continuously changing envi
