Job Description: The incumbent will provide case management, assessment and counseling services to adults who are experiencing changes in their quality of life due to the deterioration of their mental health. Working collaboratively with a number of MHAS providers and the community, the primary responsibility of the incumbent will be to provide assertive outreach case management services in a variety of community settings to a wide range of adult clients. The incumbent will work as part of a team focused on providing psychosocial case management supports to persons in need of assistance with understanding their illness, recovery planning, housing, ongoing engagement and assessment, coordination of services and supports and managing crisis as relevant. Depending on client demand the incumbent may be assigned to provide counseling supports to other MHAS Teams as required.  
Required Qualifications

Bachelor Degree in Social Work
Valid Class 5 driver's license
Registered or eligible for registration with the Saskatchewan Association of Social Workers (SASW)
Experience
A minimum of one year of supervised clinical counseling experience working with individuals, families and groups, preferably in a mental health and addictions program or family services setting.
Experience working with community groups and affiliated services is an asset
