Job Description: Reporting to the Manager of Chronic Disease Management, the CDM dietitian is responsible for developing, implementing and evaluating specific individual and group educational activities for clients, families, health professionals and students in the area of chronic diseases specifically diabetes, obesity, and cardiovascular disease in clinic-based and community-based outreach programs. The incumbent is responsible for managing client nutritional care including assessment, development and implementation of nutritional care plans, diet counselling, evaluation and follow-up. The incumbent participates in quality monitoring and research related to the CDM program area.
Required Qualifications

Bachelor Degree in Nutrition
Accredited Dietetic Internship
Experience
1 year clinical experience counselling clients/families in the area of chronic disease management including diabetes required.  1 year experience in adult education and group facilitation including teaching/counseling, facilitating and leading group activities required.  1 year experience in program/resource development related to needs assessment, planning, program design, implementation and evaluation required.  
Knowledge, Skills and Abilities

Ability to work independently and as a member of a multi-disciplinary team
Ability to communicate with and maintain an effective working relationship with clients from diverse social, economic and cultural backgrounds
Ability to competently use computers and software applications as required by the position
Ability to develop, evaluate and modify programming provided to clients
Ability to travel both within the region and provincially
