Description

SIGN operates a co-ed group home licensed for five adolescents. The home provides a residential group care program to assist the youth in understanding and resolving personal and family issues. The objective of the home is to prepare youth to successfully return to the care of parents or guardians or to provide support to the youth leading to independent living.
The role of the Primary Youth Care Worker is to work with the Program Manager, Case Management Supervisor and other staff in fulfilling the overall objectives of the home. The position will assist in the development and implementation quality programming that will enhance the personal, educational, emotional, and social development of the youth living in the home.  
Duties Include:
-    Overall case management for one or more youth.
-    Maintaining youth files with all relevant reports such as:
-    Intake reports
-    Admission reports
-    Case conferences and updates reports
-    Discharge reports
-    Assess, evaluate, plan, and revise youth care/support as directed by the AGH Program Manager or AGH Case Management Supervisor.
-    Demonstrate cooperation among other staff members in order to ensure efficiency of care and support for youth
-    Protect the health and safety of others by adopting safe work practices and reporting unsafe conditions immediately
-    Ensure familiarity with the case plans as developed for each resident and observing any special considerations or following any special instructions indicated
-    Complete the day's reports and file as needed and where needed
Qualifications:
Bachelor of Social Work Degree preferred but equivalent in education, training and experience will be considered.  Experience working with youth in a residential setting. Experience in the development and implementation of programming related to the various needs of youth.  Ability to work effectively as a team member.  Strong verbal and written communication skills.
Approx. 52.5 hours every 4 weeks.  On call as required.
