Job Description: Under the direction of the Director of Community Services, within the context of client and family centered care model and in accordance with the vision and values of Athabasca Health Authority, this position is responsible for providing
consultative social services for families with children who are experiencing complicated mental health needs. The Jordans Principle Health Facility Navigator will work with families: to help them at many points along the health care
continuum. This may include finding specialists for their child, understanding treatment and care options, understanding insurance benefits, accompanying them to visits, serving as coach and quarterback of their health care team, working with
family members and caregivers, mobilizing resources, managing medical paperwork and finding shelter/housing options.

Jordans Principle Patient Navigator will support the restoration, maintenance and enhancement of social functioning by mobilizing strengths and supporting coping capacities of families.
The Patient Navigator serves as a role model for at-risk clients and families, to enhance their opportunities for success



Provide education and tools related to wellness and subjective well-being

Play a capacity-building role in working with children, youth, families and the community.

Within our community and in support of Athabasca Health Authority Team the Patient Navigator Patient/Family
Education: educate patients and families to facilitate understanding of hospital/clinical services;

Increase client/patient/family understanding of illness/disability on relationships; and facilitate life transitions when health conditions require a modified lifestyle.

Locate resources; identify options and available supports; facilitate referrals and app
