Facility: 315 Avenue M South
Job Description: The Case Manager provides social case management support as a primary care provider to the client population.  The population targeted for this project are those who have difficulty engaging or have disengaged from care, involved in substance abuse and who are impacted by substance use disorder and Mental Health issues.
Required Qualifications

Bachelor Degree in Social Work
Registered or eligible for registration with the Saskatchewan Association of Social Workers (SASW)
Experience
1 year experience working with marginalized populations in a health related field.
Knowledge, Skills and Abilities

Advanced problem solving skills
Ability to work independently and as a member of a multi-disciplinary team
Ability to adapt to changing conditions
Ability to assist and develop policies and procedures according to best practices
Ability to communicate with and maintain an effective working relationship with clients from diverse social, economic and cultural backgrounds
Ability to competently use computers and software applications as required by the position
Ability to travel both within the region and provincially
Advanced assessment skills
Advanced mediation skills
Communication skills to transfer knowledge/skills
Knowledge of diverse health issues, culture and traditions
Knowledge of the health region and community resources
