Facility: MEADOW LAKE HOME CARE
Job Description: The Palliative Care Home Care Nurse is accountable for the day-to-day coordination and case management of all palliative care clients throughout the continuum of care in Meadow Lake, Goodsoil and Loon Lake area. Using a case management model and in partnership with members of the continuum of care, the main function of the incumbent is to establish the direction and coordination of an integrated Palliative Care Program. This position will assist the client and family to obtain a high quality of care by jointly developing, implementing and evaluating a plan of care that meets the needs of the client and their family/supporters.
Required Qualifications

Bachelor Degree/Diploma in Nursing
Registered or eligible for registration with the Saskatchewan Registered Nurses Association (SRNA)
Valid Class 5 driver's license
Knowledge, Skills and Abilities

Advanced communication skills
Advanced interpersonal skills
Ability to adapt to changing conditions
Ability to travel both within the region and provincially
Advanced assessment skills
Working knowledge of the RAI-HC Assessment Tool

Additional Home Care and Palliative Care training /experience would be considered an asset.
Expected Start Date: January 28, 2019
FTE: 1.00

Hours of Work: In a 6 week rotation: 28 shifts of 8.00 hours
