Description

: The Program Coordinator is responsible for the development, implementation, and evaluation of programs designed to support adults with developmental and intellectual disabilities who live and work with the Langenburg & District Activity Centre. This PC will have the expertise to provide behavioral analysis skills, the ability to develop behavior support plans and train interventions and protocols to ensure the quality implementation of positive behavior supports to front line staff.
Core Responsibilities include but are not limited to:
    Creates, plans, assesses and coordinates Person Centered Plans and Planning meetings annually, for all participants in the group homes and activity center associated with the Langenburg and District Activity Centre
    Consults with Support Facilitator in order to develop and coordinate quality individual and group programs that align with participants Person Centered Plans and aims to increase quality of life
    Completes quarterly reports and all other government documentation for participants including financial aid (SAID etc.)
    Provides hands-on client service delivery on site and ensures Day Program Support staff schedules are followed
    Delivers training to new and current staff in the homes and activity center in CPP&SP, Level1 & 2, Abuse Policy, TLR, and medication administration training.
