Job Description: Reporting to the Manager of Chronic Disease Management, the CDM dietitian is responsible for developing, implementing and evaluating specific individual and group educational activities for clients, families, health professionals and students in the area of chronic diseases specifically diabetes and cardiovascular disease in clinic-based and community-based outreach programs. The incumbent is responsible for managing client nutritional care including assessment, development and implementation of nutritional care plans, diet counselling, evaluation and follow-up. The incumbent participates in quality monitoring and research related to the CDM program area.
Required Qualifications

Bachelor Degree in Nutrition
Accredited Dietetic Internship
Registered or eligible for registration with the Dietitians of Canada
Registered or eligible for registration with the Saskatchewan Dietitians Association
Other Information

Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement.  HSAS seniority within the Saskatchewan Health Authority will be considered under the Selection Criteria.
