The Yorkton Tribal Council is seeking one (1) highly motivated individual for the position of Registered Diabetes Nurse Educator in the Health and Social Development Department.
This position is a part of the Diabetes Education Program, which is responsible for the ongoing planning, direction and evaluation of the Aboriginal Diabetes Initiative at YTC. As a member of this team, this position helps to conduct diabetes workshops, organize events, and support clients of the member First Nations communities. The Diabetes Education Program consists of a Diabetes Nurse Educator and a Chronic Disease Dietitian, and reports to the Director of Health and Social Development.
Duties:
Plan, implement, and evaluate educational workshops, presentations, and events
Counsel clients and support clients, conduct supportive measures as needed (e.g. home visits)
Conduct foot care clinics
Represent YTC on health committees and other duties assigned by YTC Tribal Chief/Program Director
Attend professional development sessions to maintain and enhance professional skills
Provide opportunities for YTC staff and member First Nations front-line workers to upgrade their knowledge on chronic diseases (e.g. provide presentation on the updated Canadian Diabetes Practice Guidelines)
Qualifications:
Bachelors Degree in nursing RN/BN
Certified Diabetes Educator, in process of obtaining CDE designation, or commitment to obtaining CDE
Prior experience with counseling, assessment, and goal setting
Prior experience working in the field of diabetes is considered an asset
Prior experience in group facilitation and knowledge of adult education methods is considered an asset
Awareness of community resources for clients with diabetes or commitment to learn these resources
Sensitivity and awareness of cultural, racial, economic, and socially diverse environments
