SPECIAL CARE COORDINATOR

Lac La Ronge Indian Band Health Services is now accepting applications for a Permanent Full Time Special Care Coordinator based out of the Elder Catherine Charles Long Term Care Home on Far Reserve, in the community of La Ronge.  Under the direction of the Home & Community Nursing Manager, the Special Care Coordinator is accountable for quality assurance, risk management, implementation of strategic priorities, and employee and client safety.

Duties:
    Design, develop and ensure effective program delivery;
    Develop and enhance service delivery models;
    Develop proposals, reports and track statistical information;
    Active participation in the development and implementation of the community health plan, strategic plan and evaluations;
    Development of annual budgets and work plans
    Assess needs of residents on an on-going basis;
    Other duties as assigned.

Qualifications:
    Current Registration with the Saskatchewan Association of Licensed Practical Nurses (SALPN);
    Degree or Diploma in Special Care/Assisted Living preferred;
    3-5 years supervisory/management experience;
    Acute or home care experience is preferred;
    Must be self-directed and demonstrate organizational skills;
    Current Criminal Record Clearance, Vulnerable Sector Screening and Driverss Abstract;
    Mandatory Valid driverss license & good driving record.
