Description

The Outreach Worker is responsible the distribution of information to the client communities regarding the support services, resources and interventions available for youth at the White Buffalo Youth Treatment Centre.  Outreach services will be targeting at risk youth or who are already leading at risk lifestyles.

DUTIES AND RESPONSIBILITIES

Community Outreach Services
Provide promotional presentations, conferences, workshops, and tours, etc., on WBYTC and its services to First nation communities, referral agencies/organizations, and other interested parties.
Input of data into approved reporting system to meet all necessary statistical reporting requirements as required by various stakeholders including providing quarterly report statistics for Executive Director and Health Canada.
Create and provide educational presentations based on community substance issues (e.g., marijuana, solvents, drugs, etc.) to schools and community groups.

Client Intake Services
Reviews and updates all forms relevant to the client application process (e.g. intake package, permission forms, waivers, etc.)
Assists with filling out intake package  completion of required forms and answering inquiries

Client Assessment Services
Provides a preliminary assessment of youth applicants for suitability, recommend for intake, and, determination of intake dates
Utilize database for client intake and aftercare and analyzation of statistical data for the centre in consultation with the Clinical Supervisor and Executive Director.

Client Care Services
Assists with arranging detoxification for youth prior to intake if required
Provides safe transportation for youth as necessary upon admission and discharge.
Establishes an aftercare plan for youth who have completed the program or are scheduled to leave the Centre within the last month prior to departure
