Job Description: Reporting to the Manager of Chronic Disease Management, the incumbent has primary responsibility for Chronic Disease Management Programs in the area of Diabetes Physician Practice Support, assisting with development/ coordination/conduct of inpatient and outpatient programs and clinics; utilizing established criteria to assess and plan each client's self-management care needs, providing appropriate intervention to ensure timely access to care; provide efficient and effective individual self-management education/counseling for patients/families/caretakers in all aspect of Diabetes management care and treatment. This will include liaison with families, physicians, dietitians, staff nurses and community agencies.
Required Qualifications
Bachelor Degree in Nursing
Basic Life Support (BLS)
Registered or eligible for registration with the Saskatchewan Registered Nurses Association (SRNA)
Current CPR Certification
Must possess a valid Saskatchewan drivers license and have access to a reliable vehicle
RN Special Practice for Insulin Dose Adjustment
Membership or eligible for membership in the Diabetes DES
Experience
Requires 3 years related clinical nursing experience in a family practice setting with patients and families with chronic disease, in particular, diabetes. Requires a certified Diabetes Educator with demonstrated skills and abilities related to diabetes in family practice setting. 1 year experience in adult education and group facilitation related to the area of diabetes disease management required. 1 year experience in program/resource development related to needs assessment, planning, program design, implementation and evaluation, teaching/counselling, facilitation skill
