Description

The HIV AIDS Response Team is a dynamic team that delivers rapid wrap around in- patient support to persons living with HIV. This support addresses existing gaps and barriers both medical and social to allow for stabilization and improved health and social outcomes for persons living with HIV.Work with people living with HIV/AIDS who are affected by mental illness, addictions, poverty, and other psycho-social factors.Work with patients on individual goals, life skills and care plans aimed at management and recovery of addiction utilizing a harm reduction approach.Coordination of psycho social and medical components of individual care plans, with emphasis on addiction, facilitating referrals to appropriate agencies.Provide addiction support and appropriate agency referrals pertaining to addiction utilizing a harm reduction approach.Provide therapeutic counselling/assistance to clients to complete treatment of acute illness with emphasis on addressing addiction ensure outcomes are recorded, measured and documented in individual charts.
Qualifications
Cultural sensitivity of First Nations issues
and mental health and addictions. Extensive knowledge of community and addiction resources and how to access and advocate for these services. Ability to communicate with and maintain an effective working relationship with clients from diverse social, economic and cultural backgrounds.Ability to communicate with and maintain an effective working relationship with other members of team.An understanding of the concomitant issues related to HIV and addiction.Extensive knowledge and experience with harm reduction including TVIC,Conflict resolution and interpersonal skills
Background working in Mental Health and Addictions Degree in social work, addictions diploma or related field
Valid drivers license
